Share Your Thoughts: Residential Needs Survey
Dear Friends and Families,
As a result of numerous requests for residential services, privately and publically funded, and the possibility of opening a new privately funded residential home in January of 2012, we are conducting a residential feasibility study to better determine the needs of the families in our community. We started our services over 10 years ago with a survey to assess the need and we need your participation now in order to better plan our services for the future.
We currently offer a variety of services including different models for community living, day activities at Creative Gifts; Peaceful Living’s community center, Habilitation (in home care), Friendship Connection and Congregational support. We serve a total of 65 people every day and work with 35 volunteers on a monthly basis. We also are working closely with 23 area congregations, supporting them so they can better respond to people with disabilities, as well as identifying and using their gifts. You’re always welcome to contact us at the office to find out more information.
We are interested in knowing the activities and abilities of your family member in need of services as well as which type of residential model you and your family member would prefer. To help us ascertain this information please take a moment to complete the survey below.
